Good Evening Students and Parents/Guardians,
Warm Springs Middle School will be conducting SBIRT (Screening, Brief Intervention, and Referral to Treatment) screens for risk factors of mental health and substance use on Monday, September 20th, 2021. This process helps the mental health team to target student's social and emotional needs and provide intervention in the most effective way. We understand the importance of mental health in these challenging times. Upon completion of the screening assessments, parents/guardians may be contact regarding results and information regarding small groups.
Please complete the electronic form and turn into school by Thursday September 16th, 2021, you can access this form via PDF fillable file on the Warm Springs Middle School webpage and the WSMS Facebook Page. Please complete an Opt-out form only if you DO NOT WANT YOUR STUDENT TO PARTICIPATE in either the mental health and/or substance abuse risk factors screening and email the form back to school social worker Catherine Wood at catherine.wood@k12.wv.us.
